You can call one of our Benefits Counselors at the Benefits Service Center to learn more about your benefits and complete your enrollment process by either electing, changing, or waiving benefits.

Mid-Year Family Status Change

If you experience a Qualifying Life Event (QLE) during the plan year you may be able to add or make changes to your current benefit elections. You must notify the JM Bozeman Benefits Department within 30 days of the change.

VOLUNTARY BENEFITS

Voluntary Benefits are designed to provide additional cash flow to assist with out of pocket medical costs and other bills based on individual concerns and needs.

ACCIDENT INSURANCE

Offsets the cost of unexpected expenses that result from accidents that occur everyday. It also features a Wellness Benefit payable every year while coverage is in force.

CRITICAL ILLNESS

Provides immediate financial relief from overwhelming expenses of a serious illness. It pays a lump-sum cash benefit when you are diagnosed with a covered illness easing your financial worries.

HOSPTAL INDEMNITY

complement your health insurance to help you pay for the costs associated with a hospital stay. It can also provide funds that can be used to help pay the out-of-pocket expenses your medical plan may not cover, such as co-insurance, copays, and deductibles.

LIfeTime Benefit Term

Provides protection for you and your loved ones in the event of untimely death and while you are living too, if you need home health care, assisted living, or nursing care. This benefit has a Guaranteed Rate till age 100.
